This form must be notarized.  For your convenience, free notaries are available at the school and church offices.

This form must be completed by the parent or guardian and handed to a TCS Administration Staff Member ONLY.

2009 - 2010 Extended Care Registration Form
Acceptance is dependent on available space.
An $80 registration fee must accompany the registration form.

2009 - 2010 Volunteer Packet
This packet contains the instructions and forms needed to volunteer at TCS.
